Haematological disorders, opportunistic infections and fevers: 3 case reports Three patients with angioimmunoblastic T-cell lymphoma (AITL) developed opportunistic infections, fevers and haematological disorders during treatment with alemtuzumab. A 73-year-old woman commenced IV alemtuzumab 30mg three times weekly in June 2007, after previous chemotherapy regimens were unsuccessful. Prophylaxis was also initiated with cotrimoxazole [trimethoprim/ sulfamethoxazole], fluconazole and aciclovir. She developed fevers within 3 weeks, and reactivated cytomegalovirus was confirmed. Alemtuzumab was continued for 1 week with concomitant valganciclovir. CMV was suppressed successfully, but she subsequently developed neutropenia and thrombocytopenia. In June 2009, alemtuzumab was restarted with prednisone for a AITL relapse. She developed febrile neutropenia and CMV, and subsequently died of disease progression. A 73-year-old man, who had previously received chemotherapy, started a 7-week course of IV alemtuzumab 30mg thrice weekly in June 2007 for relapsed AITL. Pancytopenia developed 3 months after completing treatment, followed by agranulocytosis. Treatment with immune globulin was successful. He also developed aspergillus pneumonia [time to reaction onset not stated], which was also successfully managed. A 62-year-old woman received a 6-week course of IV alemtuzumab 30mg three times weekly after previous chemotherapy was ineffective. Famciclovir prophylaxis was also initiated. Fevers developed and reactivated CMV was confirmed [time to reaction onset not stated]. Valganciclovir was started, but further complications included pancytopenia and legionella pneumonia. Pancytopenia spontaneously resolved, and her other symptoms were managed appropriately. Amengual JE, et al.
