QT interval prolongation in an elderly patient: case report A 65-year-old man developed QT prolongation during treatment with tacrolimus for post-transplant immunosuppression, against a background of amiodarone use for atrial fibrillation. He had been taking amiodarone 200 mg/day for probable atrial fibrillation since a coronary artery bypass graft five years earlier. Oral tacrolimus 3 mg twice daily was initiated after a kidney transplant. On postoperative day 1, his ECG revealed a corrected QT interval of 535 msec. The man received IV magnesium but showed no improvement, so amiodarone was discontinued on day 2 postoperatively. His QT interval gradually improved and he was discharged on postoperative day 5 with a QTc of 493 msec, receiving 3 mg/day of tacrolimus. Author comment: "[T]he amiodarone-tacrolimus interaction led to a sudden and marked prolongation of the QT interval when tacrolimus was commenced and improved when the amiodarone was discontinued."
