Death secondary to drug interaction following medication error: case report A female patient [age not stated] died following concomitant administration of amitriptyline and morphine after dispensing morphine without reviewing prescription history [routes, dosages and indication not stated]. The patient was dispensed with amitriptyline initially. Thereafter, morphine was dispensed to her 19 days later without reviewing her prescription history (drug dispensing error), and she started taking amitriptyline and morphine. Six days after dispensing the morphine, she died due to drug interaction between amitriptyline and morphine. As per the federal Omnibus Budget Reconciliation Act (OBRA) of 1990 the pharmacy staff should review the patient’s prescription record to identify potential drug-drug interactions and according to law the pharmacy staff should refuse to dispense the morphine after dispensing the amitriptyline. Based on this act and clinical presentation it was concluded that she died due to drug interaction of amitriptyline and morphine caused by drug dispensing error. Fink JL.
