Lack of efficacy: case report A 35-year-old woman exhibited lack of efficacy during treatment with lamotrigine, levetiracetam, topiramate, zonisamide and perampanel for tonic-clonic seizure. The proband woman had her first tonic-clonic seizure at the age of 16 years. After a six-year seizure-free period on carbamazepine, the seizures recurred. Later, at the age of 35, the attacks were characterized by complex auditory hallucinations associated with aphasia and fear and sometimes loss of contact. Her seizures increased over time and were refractory to multiple antiepileptic-drugs, including lamotrigine, levetiracetam, topiramate, zonisamide and perampanel [routes and dosages not stated]. Later, she was diagnosed with autosomal dominant lateral temporal lobe epilepsy associated with a novel RELN mutation, p.Arg2011Cys. Michelucci R, et al.
