Antipsychotics/clonidine Various toxicities and off label use: case report An approximately 35-years-old woman developed galactorrhoea, weight gain and extrapyramidal side effects during treatment with haloperidol, olanzapine, risperidone and clozapine for schizophrenia. Later, she showed weight gain during treatment with paliperidone and aripiprazole. Additionally, she received off-label treatment with clonidine for schizophrenia with no associated ADR [routes, duration of treatments to reactions onset and outcomes not stated; not all dosages stated]. The woman presented at the age of 42 years with the complaints of feeling uneasy, being controlled, hearing voices, anhedonia and pessimism. She reported of hearing voice of an unknown man, and that person had taken control of her left hand and eye, while demeaning and insulting her. The signs of agitation, anxiety, insomnia and depression secondary to hallucinations and delusions were observed. She had no insight. Her complaints had started 7 years prior (at the age of approximately 35 years) with delusion, hearing voices, being controlled and of persecution. Before the presentation, she received treatment with haloperidol, olanzapine, risperidone and clozapine, which were discontinued due to weight gain, galactorrhoea and extrapyramidal side effects. Based on the clinical presentation, she was diagnosed with schizophrenia. Subsequently, she started receiving paliperidone 6mg, which was gradually increased to 12mg. At 8 weeks follow-up, she showed weight gain (6kg) associated with paliperidone, which led to decline in compliance to treatment. Subsequently, her treatment was changed to aripiprazole [initial dose not stated]. After 4 weeks, the dose of aripiprazole was changed to 30mg, which was administered for 12 weeks. An improvement was observed in her symptoms; However, the compliance to the treatment was deteriorated due to continued weight gain associated with aripiprazole. Thereafter, the woman received off-label treatment with clonidine. After 7 days of treatment, a dramatic improvement was observed in her auditory hallucinations. At 6 months follow-up, her auditory hallucinations completely resolved.
