First report of an interaction, leading to severe global dehydration in an elderly patient: case report A 65-year-old man developed severe global dehydration and died after concurrently receiving bexarotene and aprepitant [routes not stated]. In November 2009, the man received interferon-α-2a and bexarotene 300 mg/m2/day for an erythrodermic Sezary syndrome. In September 2011, he received aprepitant 80 mg/day for chronic pruritus. In November, he experienced generalised asthenia, with aphagia and anorexia, which led to a 15 kg weight loss over a 3-week period. He was subsequently diagnosed with severe dehydration, hypernatraemia and acute renal insufficiency. He died in December, despite symptomatic treatment. Editorial comment: A search of AdisBase, Medline and Embase did not reveal any previous case reports of an interaction between aprepitant and bexarotene. The WHO ADR database did not contain any reports involving the concomitant use of aprepitant and bexarotene.
