Myocardial infarction: case report A 42-year-old man, who had woken up with a headache, ingested a dose of an OTC effervescent cold remedy containing aspirin 325mg, chlorphenamine 2mg, dextromethorphan 10mg and phenylephrine 7.8mg. Twenty minutes later, he developed chest pains and his wife called an ambulance. A 12-lead ECG revealed ST segment elevation and he was admitted to a coronary care unit. Catheterisation showed diffuse spasm of the right coronary artery. Following administration of intracoronary nitrate and passage of a guide wire, his artery appeared normal. His troponin T level, measured at 12 hours after the onset of chest pain, confirmed MI. Three days later, he was discharged on advice to avoid ephedrine-containing agents.
