DRESS syndrome: case report A 58-year-old woman developed DRESS syndrome during treatment with aspirin, furosemide and metoprolol. The woman underwent elective coronary artery bypass grafting. She was discharged on post-operative day 6 following CABG on furosemide, aspirin [acetylsalicylic acid] and metoprololol [routes, dosages and specific indications not stated]. Two weeks later, she was re-admitted with vomiting and nausea. On admission, laboratory results revealed mild neutropenia, low eosinophils, high Creactive protein and hepatic dysfunction. Her body temperature was 37°C, blood pressure was 140/90mm Hg and pulse rate was 96 beats/minute. After 2 days, a maculopapular rash emerged on the face and also there was associated lethargy and fever (39°C). She was initiated on unspecified empirical antibiotic and antiviral treatments while awaiting culture results. Her rash deteriorated in the next 2 days, the rash turned to erythroderma, erythematous rash was on the extremities and trunk, a facial dermatitis and a widespread maculopapular rash on the trunk, lower extremities and arms had begun, dyspnoea and hypoxia started. Her fever persisted, and she developed pancytopenia. The woman was transferred to the ICU, and she received specific treatment (prednisone and immune globulin) due to a suspicion of DRESS syndorme. She was intubated because of dyspnoea and hypoxia. Laboratory results revealed anaemia, deep neutropenia, high C-reactive protein and thrombocytopenia. Her condition rapidly aggravated with haemodynamic instability, and she died on the seventh day of ICU admission. She was considered to be a probable case of DRESS with a RegiSCAR score of 5.
