Rhabdomyolysis: case report

A 22-year-old woman developed rhabdomyolysis following treatment with β-Alanine/caffeine/citrulline-malate/theanine/tyrosine as a pre-exercise supplement.* The woman, whose medical history was notable for endometriosis, presented to the hospital with nausea, myalgia, oliguria and vomiting. Prior to exercise, she ingested β-Alanine/caffeine/citrulline-malate/theanine/tyrosine [Alani-NU] supplement [dosage not stated] and then performed 30 minutes of exercise. The following morning, she developed lower extremity weakness and myalgia. Hence, the woman had received the treatment with naproxen for 2 days. On day 3 after her workout, she had nausea, decreased urine output and darkening of urine. Upon presentation, a laboratory test was performed which showed creatinine kinase of 194000 UI/L, aspartate aminotransferase (AST) of 1900 U/L, alanine aminotransferase (ALT) of 500 U/L, potassium of 8.2 mEq/L and creatinine of 7.0 mg/dL. Subsequent urinalysis revealed 8 RBCs. Based on laboratory investigation, rhabdomyolysis with acute kidney injury was diagnosed. Subsequently, she started receiving treatment with IV crystalloid. However, her hyperkalemia did not improve. Thereafter, due to signs of volume overload, the intermittent haemodialysis was initiated. Following the corrective measures, her condition improved. On hospital day 11, she was discharged with normal liver enzymes, but with a creatinine of 4.2 mg/dL. * Country of occurrence, reporter country and primary source country not stated.
