Health systems across the world face a continuing challenge to improve safety for the patients in their care. As systems and treatment regimes become ever more complex and patients present with more co-morbidities, this challenge increases. Many improvements have been made and continue through the efforts of dedicated professionals. Often, these are quite specific and the result of a focus on a particular condition or issue. More worryingly and as Chap. 1 of this text suggests, the overall levels of experienced adverse incidents have not decreased to the levels that might have been anticipated. In reality, the rate of incidents has remained at a fairly constant level for many years. This picture accords with my own previous experience of clinical practice. Retrospective investigations, many with worthy intent, would seek to highlight the cause or trail of events. But over the years, it seemed a similar, marginally different pattern of events would be described. It f
