Horner’s syndrome: case report A 33-year-old woman developed persistent, unilateral Horner’s syndrome after the administration of bupivacaine and fentanyl [dosages not stated]. The woman received an epidural block with bupivacaine 0.125% and fentanyl 2 mcg/mL during labour and underwent a caesarean section for delivery. Upon administration of the bupivacaine and fentanyl, she experienced immediate right-sided back pain. Approximately 12 hours after the administration and postoperatively, right-sided blepharoptosis was noticed [exact time to reaction onset unknown]. She presented with persistent blepharoptosis 9 weeks after her caesarean section. The patient’s cranial nerves and neurological exam were normal. She was diagnosed with Horner’s syndrome. Upon examination, she had right-sided blepharoptosis of 1mm, right-sided proptosis of 2mm and anisocoria. There was no pupillary response in her right-eye after the installation of cocaine 2%.
