Toxicity: case report A 36-year-old woman was found dead following an apparent suicide; her death was certified as caffeine toxicity. The woman was found lying dead on her hotel bed; it was an apparent suicide and a note of intent was found on scene. A drinking glass containing pink powder residue was on the floor. A bottle of clonazepam, which had been prescribed to her, was noted, and the pill count correlated with the fill date of prescription. An autopsy revealed diffuse pulmonary oedema and apparent medication sediment in her gastric contents. A qualitative drug screen was positive for caffeine and salicylate. Her clonazepam concentration was within the therapeutic range, and her salicylate concentration was below the reference range. A test of the glass residue was positive for caffeine, and she had a significantly increased caffeine concentration of 240.0 µg/mL. Her death was certified as caffeine toxicity [details of caffeine administration not stated]. Author comment: "This case emphasizes the importance of post-mortem drug testing, which should continue to include screening for caffeine and its metabolites." Katzung KG, et al.
