Portal pneumatosis and rectal mucositis in an elderly patient: case report A 73-year-old man developed portal pneumatosis and rectal mucositis following chemoradiotherapy with capecitabine and oxaliplatin [durations of treatment not stated]. The man presented with abdominal pain, fever, diarrhoea and tenesmus after completing chemoradiotherapy. Ten days earlier, he was treated for advanced rectal cancer with IV oxaliplatin 60 mg/m2 [frequency not stated] and capecitabine 825 mg/m2 twice daily [route not stated], together with radiotherapy. On presentation, he was hypotensive and tachycardic, while tests revealed leucopenia and elevated inflammatory markers. CT scan revealed acute inflammation at the recto-sigmoid junction and gas in the portal venous circulation. The man was admitted to the ICU, and received antibacterials. He then underwent a loop ileostomy for faecal diversion. He clinically recovered, with a CT scan revealing resolution of the abnormality 2 months later.
