Splenic infarction: case report A 43-year-old man experienced splenic infarction following injection of cyanoacrylate for fundal varices [dosage not stated]. The man, who had liver cirrhosis and hepatitis B and had undergone regular variceal therapy for fundal varices, presented for secondary prophylaxis. Small oesophageal varices and large fundal varices were revealed by endoscopy and he received two injections of 2mL of cyanoacrylate ‘Histoacryl’ diluted with ‘Lipiodol’ into the fundal varices. Two days later, he experienced fever and abdominal pain and his WBC count increased to 14 × 103 /µL. Abdominal CT angiography showed cyanoacrylate remnants in his splenic vein and nonenhancing splenic parenchyma; findings indicated cyanoacrylate-induced splenic infarction. The man received IV antibacterials and supportive care. His fever and abdominal pain gradually abated and his WBC count returned to normal. On hospital day 20, he was discharged without symptoms. Repeat CT scanning revealed persistent, large, hypoattenuated splenic lesions. However, during a 9-month follow-up period, he remained asymptomatic. Kim J, et al.
