Various toxicities: case report An 11-year-old girl developed fever, rash, haematochezia and tinnitus during treatment with ethionamide, cycloserine or kanamycin for rifampicin-resistant tuberculosis [routes not stated]. The girl, who had Turner’s syndrome, presented with cough, fever and episodes of haemoptysis. Hence, the GeneXpert sputum test was performed, which revealed rifampicin-resistant tuberculosis. Hence, she was hospitalised in Indonesia and scheduled for a shorter treatment regimen. Therefore, she initially received kanamycin 200mg injection and moxifloxacin as first-line therapy, and after every two days, new treatment drugs introduced for tolerability testing. In the same sequence, she received isoniazid, pyrazinamide, ethambutol, ethionamide 250mg, cycloserine 250mg and clofazimine. However, she developed fever and rash due to ethionamide, haematochezia and severe fever due to cycloserine and tinnitus due to kanamycin [times to reactions onsets not stated]. Of note, she tolerated the treatment with moxifloxacin, isoniazid, pyrazinamide and ethambutol. Hence, the girl again hospitalised, and treatment with all drugs was interrupted. After the resolution of adverse reactions, treatment with previously tolerated drugs was re-started, along with the addition of delamanid. She tolerated the new treatment regimen and had an improvement in her weight. Krisnanda A.
