Thrombus formation after switching from warfarin in an elderly patient: case report A 78-year-old man with hypertension underwent coronary angioplasty and stent implantation. Two years later, a routine examination revealed atrial fibrillation. A left atrial thrombus was seen on transeosophageal echocardiogram, and he began receiving warfarin. After one year, a repeat echocardiogram showed thrombus resolution. Based on results from the RE-LY study, he was switched to dabigatran etexilate 110mg twice daily [route not stated]. Eight months later, a routine echocardiogram revealed a 2.1 × 2.5cm left atrial thrombus, and dabigatran etexilate was replaced with warfarin [duration of treatment to reaction onset and outcome not stated].
