Recurrence of nodular foreign body reactions and late inflammatory response syndrome on dose reduction, headache and diarrhoea: case report A 37-year-old woman experienced recurrence of nodular foreign body reaction and late inflammatory response syndrome (LIRS) on tapering methylprednisolone, and she developed headache and diarrhoea during treatment with dimethyl fumarate for granulomatous foreign body reaction to cosmetic soft tissue filler with LIRS [not all routes stated]. The woman presented to a clinic with a painful, oedematous facial swelling, accompanied by a burning sensation of her face for 2 weeks. Seventeen years earlier, she had experienced a firm permanent induration of both cheeks a few months after injection of dermal filler into her cheeks and glabella. Subsequent analyses identified a granulomatous foreign body reaction. Therefore, she started receiving oral methylprednisolone 1 mg/kg, leading to a marked decrease in the pain and oedema within a few days. However, her symptoms promptly reoccurred on dose reduction. Following a proposed treatment algorithm for nodular foreign body reactions with LIRS, she resumed systemic methylprednisolone, combined with ciprofloxacin and clindamycin for 4 weeks. Further, in an effort to reduce glucocorticoid intake, off-label hydroxychloroquine 200 mg/day was added; however, it did not prevent another flare-up of her symptoms on tapering of methylprednisolone. Thereafter, she started receiving dimethyl fumarate 120 mg/day alongside low-dose prednisolone, after which her swelling and oedema stably reduced. However, she subsequently developed headache and diarrhoea secondary to dimethyl fumarate, which necessitated therapy discontinuation [duration of treatment to reactions onsets and outcomes not stated].
