Isolated accommodation spasm: case report A 13-year-old girl developed isolated accommodation spasm following diphtheria-tetanus vaccination. The girl was admitted to the hospital with a chief complaint of decreased vision. She reported that her decreased vision was started following the diphtheria-tetanus vaccination at school [route and dosage not stated]. She did not have any history of any illness or drug use. An ophthalmological evaluation showed visual acuity with Snellen chart was in the level of 0.1 in the left eye and hand motion in the right eye. A near and far orthophoria was noted with natural eye movement in every direction. Miosis was absent. Based on the presenting symptoms and ophthalmological evaluation, a diagnosis of isolated accommodation spasm following diphtheria-tetanus vaccination was made [duration of treatment to reaction onset not stated]. The girl was treated with cyclopentolate drops in both eyes. Forty-five minutes later, improvement in refraction values and visual acuity of both eyes were noted. The posterior and anterior segment examinations were naturally noted. In view of her condition, a conservative follow-up was planned. Thereafter, in a few days, her decreased vision improved. Three months later, the bestcorrected visual acuity and uncorrected visual acuity were found to be 1.0 and 0.8, respectively. The autorefraction values were improved after treated with cyclopentolate drop. A natural eye movement in every direction was noted without miosis and ocular misalignment. Batur M, et al.
