Masking of hypertension: case report A 65-year-old man exhibited masking of his underlying hypertension during treatment with doxazosin for benign prostatic hyperplasia. The man, who had benign prostatic hyperplasia, started receiving doxazosin 2mg once a day before bedtime [route not stated]. Subsequently, due to urinary retention [aetiology not stated], the dose of doxazosin was increased to 4 mg/day. Also, urethral catheter was inserted for a week. Five days before the follow-up appointment, he again experienced urinary retention. Due to refractory urinary retention, he was planned for transurethral resection of the prostate (TURP). He was hospitalised for perioperative evaluation during which, doxazosin was discontinued. On admission, the his systolic BP was around 130–150mm Hg and diastolic BP was around 80–90mm Hg. On the day of surgery, his BP was found to be 180/100mm Hg. The BP reduced to 150/90mm Hg after resting and the pulse rate was 80 beats per minute. Later, it was determined that the therapy with doxazosin had masked his underlying hypertension. Author comment: "[H]e had undiagnosed hypertension obscured by regular use of doxazosin."
