Neutropenic fever: case report A 44-year-old woman developed neutropenic fever during treatment with doxorubicin and ifosfamide for primary metastatic angiosarcoma of the pericardium. The woman, who had chest pain and shortness of breath, presented to an emergency room. A year before this presentation, she had been diagnosed with pericardial effusion. At the same time, latent tuberculosis was diagnosed by the tuberculin skin test. She had received isoniazid therapy for 6 months; however, her symptoms deteriorated over 6 months and she presented to the hospital with severe shortness of breath (current presentation). Based on further investiations, primary metastatic angiosarcoma of the pericardium was diagnosed. Therefore, she started receiving AIM 75/10,000 regimen consisting of doxorubicin 25 mg/m2 on day 1-3 and ifosfamide 2000 mg/m2 on day 1-5 [routes not stated] along with mesna and unspecified granulocyte colony-stimulating factor support. She completed 6 cycles of therapy with 25% dose reduction and minor treatment delays due to two episodes neutropenic fever [time to reaction onset and outcome not stated]. Partial response was noted with atleast 50% reduction in the tumour burden after 6 cycles; however, within 6 weeks of completing treatment, she developed cardio-respiratory failure and opted for hospice.
