Foreword

This book on edge-to-edge mitral valve repair represents a remarkable compendium because it is organized and edited by the inventor of the technique and incorporates a perspective that has been shaped by two decades of development. This path of development has been controversial, sparking debates about the optimal methods for surgical repair for mitral regurgitation (MR) and serving as the basis for the beginning of the new field of percutaneous therapy for MR. All of these issues are contained and addressed within this text, which is unique for its depth on this subject. The surgical edge-to-edge technique gave rise to percutaneous edge-to-edge with the MitraClip. Remarkably, the visibility of percutaneous repair has raised awareness of the utility of surgical edge-to-edge among the surgical community. I have been interested to see many surgeons, who were often highly critical of the edge-to-edge repair, come to adopt the use of surgical edge-to-edge when conventional annuloplasty is insufficient, with recognition that edge-to-edge can be a predictor of better surgical outcomes than isolated annuloplasty.
