Knee osteonecrosis: case report A 63-year-old man had been treated for HIV disease for 20 years and had a history of therapy-induced lipodystrophy. He received tenofovir and emtricitabine for 2 years and nevirapine for 3 years [dosages and routes not stated], and developed osteonecrosis of both knees [duration of treatment to reaction onset not clearly stated]. He first experienced knee joint pain in his left knee in 2008 and then in his right knee in July 2010. He presented with bilateral oedema of his knee joints. MRI revealed osteonecrosis of the four condyles and bone marrow oedema. His bone marrow oedema resolved in 3 months with symptomatic treatment. Author comment: "HAART-related dyslipidemia may accelerate the atherosclerotic process and occlude the bonefeeding vessels which may contribute to avascular necrosis. The presence of lipodystrophy reflects a prolonged HAART exposure". Veyrac G, et al. Knee osteonecrosis in HIV patients exposured to highly active antiretroviral therapy.
