Cross-reactivity to carbapenems presenting as acute generalised exanthematous pustulosis: case report A 58-year-old woman developed cross-reactivity to carbapenems (imipenem, ertapenem and meropenem) presenting as acute generalised exanthematous pustulosis (AGEP). The carbapenems (imipenem, ertapenem and meropenem) were administered for acute on chronic osteomyelitis [routes, dosages, durations of treatments to reactions onsets not stated]. The woman, who had multiple drug allergies, presented to hospital with acute on chronic osteomyelitis. She reported a remote history of delayed "blister-like" rash on ertapenem and a recent similar rash with desquamation on imipenem. She was hospitalised and her hospital course was complicated by delayed rash with multiple recent antibiotics [specific drug not stated]. Examinations showed confluent erythematous plaques on bilateral upper extremities and scattered pustules with colarettes of scale. A graded drug challenge to meropenem was performed to rule out IgE-mediated hypersensitivity and was initially tolerated. Afterwards, her rash worsened with appearance of pustular lesions. She reported these lesions identical to "blister-like" rash, which were observed during imipenem and ertapenem courses. Her skin biopsy were consistent with AGEP. She was diagnosed with AGEP secondary to carbapenems. The woman was suggested to avoid carbapenems [outcomes not stated].
