Anaphylactic reaction: case report A 74-year-old man developed anaphylactic reaction following treatment with ferric carboxymaltose for iron deficiency anaemia. The man, who had a history of diabetes, hypertension, chronic renal failure and coronary artery disease, was hospitalised in Turkey due to shortness of breath. On admission, he had hypertension and lung oedema. Consequently, he received IV furosemide and nitrate infusion [specific drug not stated]. Additionally, he received non-invasive ventilation. He had been receiving long-acting insulin, lercanidipin, isosorbid mononitrate and nebivolol due to his chronic diseases. The following day, he was shifted to the coronary ICU due to increased cardiac enzymes. Echocardiography showed a left ventricular ejection fraction of 40%, anterolateral hypokinesia and slight mitral and tricuspid insufficiency. Angiography showed various stenoses in his coronary arteries and right renal artery, for which percutaneous intervention was planned. Following regression of his signs of heart failure and stablisation of his condition, IV ferric carboxymaltose [Ferinject] 1000 mg/15 minute infusion was given to treat underlying iron-deficiency anaemia. His medical history was unremarkable for drug allergy or any allergic disease. Thirty seconds after the initiation of ferric carboxymaltose infusion, he felt faint. Immediately, ferric carboxymaltose infusion was terminated. Within a few minutes, he developed respiratory distress (dyspnoea and stridor). As a result, he received IV prednisolone and antihistamines [specific drug not stated]. Despite the treatment, he developed angioedema, followed by respiratory arrest. He was intubated using a video laryngoscope. Shortly thereafter, he developed cardiac arrest and cardiopulmonary resuscitation (CPR) was started.
