Hormonal replacements/lamotrigine interaction Lack of efficacy: case report

A 60-year-old woman exhibited lack of efficacy due to concomitant administration of lamotrigine for generalised epilepsy and unspecified hormone replacement therapy (HRT) for menopausal symptoms [routes and time to reactions onsets not stated]. The woman, presented to a clinic in Australia with recurrence of seizures. Recently, she had started receiving unspecified HRT for menopausal symptoms. An anamnesis showed that she had a 5-year history of difficult to control generalised epilepsy. Before the presentation, she was seizure free for 2 years on lamotrigine 300mg in the morning and 400mg in the night along with levetiracetam. She was adherent to the medications. Her previous reports showed stable serum lamotrigine trough level (greater than 11 mg/L) for over 1 year. At current presentation, her serum lamotrigine trough level was decreased to sub-therapeutic level at 7.7 mg/L from 14.7 mg/L. The decrease in lamotrigine trough level was attributed to interaction between lamotrigine and unspecified HRT. The decrease in lamotrigine trough level led to recurrent seizures, indicating lack of drug effect. This impacted her ability to drive and employment. Therefore, the woman’s lamotrigine dose was increased to 400mg in the morning and 600mg in the night. One month later, her serum trough lamotrigine level returned to therapeutic range.
