This book is the product of many hands, hearts, and minds. First and foremost, I want to acknowledge Dr. James (Jim) R. Heiby, who conceptualized, spearheaded, and led the development of this book until his retirement from the US Agency for International Development (USAID) in October 2017. For more than 30 years, he designed, oversaw, and evaluated USAID flagship projects dedicated to improving the quality of health care in low- and middle-income countries. He also provided technical guidance and encouragement to a large number of individuals and USAID- funded projects, and this book would not have been possible without his vision for what this collection of cases could be and his passion and thought leadership for this subject.
