Report Describes SCC as Cause of Flight Wheel Loss As reported at The Aviation Herald (www.avherald.com), the Italian Air Safety Board (ANSV) has released their final report regarding Air One Boeing 737-400, flight AP-2843 from Catania to Rome Fiumicino (Italy), which lost one nose wheel during takeoff on June 15, 2006. The report concludes that the probable cause was: The fracture of the left nose wheel axle’s inner cylinder due to a stress-corrosion cracking mechanism. The fracture developed from a corrosion at the outer surface of the axle in the area of most mechanical stress, developed into a pitting and further led to the fracture. The corrosion was not detected by regular maintenance inspections because the damage developed in an area not effectively checked according to maintenance procedures at that time. The fracture therefore was attributed to the corrosion prevention task card which did not require the removal of the spacer for a surface inspection of the axle. Statement Issued Regarding Maintenance of Anhydrous Ammonia Tanks In response to a recent announcement by the Iowa Department of Agriculture and Land Stewardship, American Welding & Tank—North America’s leading provider of anhydrous ammonia storage tanks—issued a statement in an effort to minimize safety risks of using and maintaining anhydrous ammonia storage tanks.
Because of the corrosive nature of anhydrous ammonia and general wear and tear on equipment, customers are encouraged to regularly inspect tanks and related equipment for any sign of damage, wear, or leak. Industry best practices are detailed to ensure leaks are immediately detected and treated. The statement concludes by assuring customers that American Welding & Tank is not aware of any incidents of serious injury sustained through a pinhole leak of anhydrous ammonia involving one of the company’s tanks, nor is the company aware of any instance in which pinhole leaks led to a rupture or larger release of anhydrous ammonia. American Welding & Tank is proud of the excellent safety record of its anhydrous ammonia tanks and wants to ensure years of continued safe use of its products. Most U.S. Gas Lines Not Inspected With Latest Technology Almost two-thirds of America’s natural gas pipelines— including the 30 in. main that exploded last September in San Bruno, CA—are susceptible to potentially deadly faults because they can not use the industry’s best technology for testing and cleaning them, federal records show. As reported at www.msnbc.com, robots known as ‘‘smart pigs’’ are the most effective way to inspect and maintain pipelines. The most advanced of them approach 95% reliability in detecting problems, the Gas Research Institute said. So-called either beca
