Abstract To investigate major accidents such as the Plasco collapse in Iran, it is essential to conduct a thorough review of such events via the Proactive Safety approach. Therefore, it is necessary to analyze and root out the event comprehensively and scientifically and utilize the identified causes as a basis for the prevention of similar accidents. In this study, the causes of Plasco building accident were investigated using Timed MTO (TM) and Accimap methods. Studying national authenticated reports and consulting and interviewing safety experts, we explored the details of Plasco accident. Then, the identified causes were first categorized at the organizational level using TM method and, to carry out a more in-depth investigation, the causes were categorized into 6 levels via the Accimap method. The findings showed that the lack of safety monitoring and inspection systems at all levels was the most root cause of the Plasco accident. Non-compliance with building safety rules and NFPA standards and poor safety culture was also identified as the main causes of failure in firefighting. Our results showed that both TM and Accimap could be used as a comprehensive tool to fully investigate the causes of accidents, particularly building accidents. Introduction Accident is one of the major human, economic, and social risk factors, especially in developing countries [1]. Meanwhile, accidents involving commercial and residential buildings have a great importance [2, 3]. Accidents in such buildings can result in heavy casualties due to the high population density [4]. Investigation of events such as the collapse of a bank building in Nigeria in 2006 (killing two persons and injuring 23 others) [5], Shanghai residential buildings accidents in 2009 [6], and the Rana Plaza building incident in Bangladesh in 2013 (killing 1134 persons) [7] show well the importance of an in-depth investigation of construction and building accidents.
