Hypersensitivity pneumonitis: case report A 58-year-old man with myeloma started treatment with lenalidomide [dosage and route not stated], doxorubicin and dexamethasone. Shortly after completing dexamethasone therapy, and after only a few days of lenalidomide treatment, he developed sudden-onset pulmonary infiltrates. He was hospitalised, lenalidomide was discontinued and he slowly recovered with antibiotic therapy. A second treatment cycle with lenalidomide was started after full recovery of his pulmonary function, but sudden pneumonitis-like symptoms recurred. Bronchoalveolar lavage showed no indication of infection, and a drug-induced reaction was suspected. He received steroids and antibiotics, and his symptoms resolved within a few days. Author comment: "Physicians prescribing lenalidomide should be aware of this rare but severe pulmonary complication, and should consider a lenalidomide induced hypersensitivity pneumonitis if patients develop a pulmonary affection during lenalidomide therapy."
