Various toxicities: case report A 30-year-old man developed generalised tonic clonic seizure, QTc interval prolongation, recurrent ventricular tachycardia and recurrent ventricular fibrillation following abuse and overdose of loperamide. The man, who had a history of epilepsy, presented with generalised tonic clonic seizures. He got intubated in emergency department secondary to low glasgow coma scale. Initially, treatment initiated for status epileptics with 4 antiepileptic drugs, but electroencephalogram was unremarkable. Later, he developed QTc interval prolongation. On anamnesis, abuse of loperamide with 100–120 tablets per day [route and dose not stated] was revealed. During the hospitalisation, he developed recurrent ventricular tachycardia and recurrent ventricular fibrillation [duration of treatment to reaction onset not stated]. The man’s treatment was supported by manual cardiopulmonary resuscitation, standard advanced cardiac life support and temporary pacemaker, which was applied and got reverted back. Fayyaz S, et al.
