First report of photodistributed dermatosis: case report A 41-year-old man developed a photodistributed dermatosis while receiving mesna. The man was admitted with a pruritic rash, which was exacerbated by sun exposure. The eruption had developed 1 month after starting mesna [dosage and route not stated] for cyclophosphamide-induced haemorrhagic cystitis. Examination revealed a papular erythematous rash, which was photodistributed on his arms, trunk and face. Tests revealed pancytopenia and normochromic normocytic anaemia. Biopsy revealed a lichenoid tissue reaction with lymphocyte exocytosis, basal cell apoptosis and a heavy inflammatory cell infiltrate in the superficial dermis. The findings were consistent with a photoinduced drug eruption. Mesna was withdrawn, and the man received promethazine, clobetasol, clobetasone and emollients. Editorial comment: A search of AdisBase, Medline, Embase and the WHO ADR database did not reveal any previous case reports of photodermatosis associated with mesna.
