Lack of drug effect: case report A 3-year-old boy exhibited lack of drug effect during treatment with nystatin and miconazole for chronic mucocutaneous candidiasis [dosages not stated]. The boy presented with recurrent oral candidiasis. His mother reported that he had a significant medical history of oral candidiasis from birth. He also had a family history of recurrent oral candidiasis (maternal aunt, grandmother, grandmother’s sister, and grandmother’s niece). Due to oral candidiasis, he was unable to eat comfortably. He received nystatin oral suspension and miconazole oral gel. However, he did not show any response to it. Subsequently, swab tests showed the presence of Candida albicans and Staphylococcus aureus. Therefore, the boy started receiving treatment with fluconazole. Subsequently, candidiasis had resolved. Later, he underwent a targeted genetic screening, which revealed a heterozygous mutation in STAT1. He was diagnosed with chronic mucocutaneous candidiasis (CMC). Sanghvi R, et al.
