Obesity affects millions of people worldwide with a negative impact on quality and duration of their lives. This condition represents a major issue in terms of public health due to the cost of the many comorbidities often associated. Moreover, obesity has a multifactorial background, thus its treatment needs a multifactorial environment with several specialists involved in the course of evaluation, treatment, and follow-up of the patients. Nevertheless, the role of the bariatric surgeon is still central in identifying a therapeutic strategy upon the suggestions provided by the multidisciplinary team. In fact, when asked to treat a condition of severe obesity, a surgeon cannot act directly on the primary cause of the obesity itself. This means that different techniques are still available to the surgeon’s choice and that the best treatment for obese patients is to “tailor” a specific procedure according to the patient’s expectations, the degree of overweight, the presence and type of comorbidities, and the expected postoperative compliance.
