NMSC observed in Hungary after immunosuppression Researchers from Hungary observed a 9.5% rate of non-melanoma skin cancers (NMSC) among patients who had undergone organ transplantation. They comment that "all currently available immunosuppression agents cause nonspecific immunosuppression" and can increase the risk of infection and malignant lesions, most commonly skin cancers. The study involved 116 White patients (median age 49.3 years) who had undergone kidney or combined kidney-pancreas transplantation, with follow-up from September 2008 to December 2009. A full skin examination for NMSC was performed by a dermatologist, and all patients also completed a standardised questionnaire to determine other risk factors, such as sun exposure and family history of cancer. In total, 16 NMSCs were detected among 11 patients (9.5%), with histological analysis confirming 13 basal cell carcinomas (81.25%) and 3 squamous cell carcinomas (19.75%). The median time since transplantation was 4.1 years. Significantly more NMSCs were observed on the head and neck than the upper limbs or trunk (13 vs 2 and 1, respectively) – "a strong indication that the sun is a factor in development of these tumors", say the researchers. Ciclosporin immunosuppression therapy was received by 10 of 11 NMSC patients (90.0%; p < 0.05), compared with 43 of 105 non-NMSC patients (41.0%); an approximately equal proportion of patients in each group had received mycophenolate mofetil therapy (82% vs 92%). Of note, 80% of transplant recipients had "never undergone a dermatologic skin cancer screening" following transplantation, comment the researchers. Fekecs T, et al.
