Nusinersen Increases in total protein, serum quotients of albumin and observation of oligoclonal IgG bands in CSF : 2 case reports In a study in which patient samples were collected from June 2017 to April 2019, a 46-year-old patient and a 42-yearold patient [sexes not stated] were described, who exhibited increases in total protein, serum quotients of albumin (Qalb) or presence of oligoclonal IgG bands (OCB) in CSF during treatment with nusinersen for spinal muscular atrophy (SMA) [dosages and durations of treatments to reactions onsets not stated]. The 46-year-old patient, who had SMA, started receiving intrathecal nusinersen injection. However, the patient exhibited high CSF protein level, which decreased over the subsequent days. Additionally, the patient showed CSF Qalb level that increased initially up to two consecutive days, but then decreased slightly on the fourth day. The 42-year-old patient, who had type III SMA, started receiving intrathecal nusinersen injection. Upon CSF analysis, transiently positive OCB was noted in this patient. It was speculated that the OCB was observed due to intrathecal synthesised anti-drug antibody (ADA). Author comment: "We suppose that the slight change in CSF total protein and Qalb is caused by repeated lumbar punctures and/or intrathecal administration of the drug". "Whether the OCB in CSF we observed in this patient may be correlates of intrathecal synthesized ADA". Wurster CD, et al.
