Hepatotoxicity: case report A woman in her early 70s [exact age not stated] developed hepatotoxicity during treatment with pazopanib for metastatic renal cell carcinoma. The woman presented in March 2011 with hematuria and lower back pain, at the age of 67 years. She had a medical history of right adrenal adenoma, diagnosed at the age of 37 years, and an ovarian cyst for which she underwent salpingo-oophorectomy at the age of 45 years. Also, she was hypertensive, which was controlled with unspecified diuretics. Later, her complete blood count showed mild anaemia. Subsequently, following various tests, she was diagnosed with renal cell carcinoma stage-pT2acN0cM0. Thereafter, in Jul 2016, chest CT scan showed multiple pulmonary nodules in the right lung. Therefore, she underwent lobectomy and right wedge resection. Then in Oct 2016, progressive lung disease was observed on the CT scan. Therefore, she started receiving treatment with oral pazopanib 800 mg/day. However, six weeks later, she developed grade 3 hepatotoxicity. Therefore, the woman’s treatment with pazopanib discontinued. In Dec 2016, after normalised biochemical parameters, pazopanib reintroduced at 400 mg/day. However, a month later (Jan 2017), she again developed grade 3 hepatotoxicity. A CT scan showed stable disease. In Jun 2017, the CT scan revealed enlargement of the subcarinal lymph node. Subsequently, she started receiving radiation therapy. Following CT scans showed a decrease of the irradiated lymph node, additionally, abscopal effect observed [final outcome of ADR not stated].
