Rheumatoid arthritis and autoimmune thyroiditis: case report A 57-year-old women developed rheumatoid arthritis (RA) [outcome not stated] and autoimmune thyroiditis while receiving treatment with peginterferon alfa-2a [route not stated] for hepatitis C infection. The woman, who had been diagnosed with chronic hepatitis C infection, started receiving treatment with 24 week course of peginterferon alfa-2a [PEGylated-IFN-alpha2a] 180 mug weekly and ribavirin 100mg daily. After 2 months of treatment, she developed symmetrical polyarthritis, with oedema and pain in the elbows, shoulders, metacarpophalangeal joints and wrists, along with prolonged morning stiffness. The musculoskeletal examination was remarkable for active synovitis of the metacarpophalangeal joints, proximal phalangeal joints, elbows and wrists. Ultrasonography of the hands showed diffuse synovitis as well as tenosynovitis of the ulnar extensor tendons in both wrists. Laboratory results showed elevated level of liver enzymes. She was diagnosed with RA based on clinical and ultrasonographic findings as well as anti-cyclic citrullinated peptide (anti-CCP) and rheumatoid factor (RF) antibody positivity. Furthermore, an autoimmune thyroiditis was noted, which evolved into hypothyroidism and treated with levothyroxine-sodium [thyroxine]. Yasmine M, et al.
