Vaccine-derived Poliovirus infection and acute flaccid paralysis: 16 case reports In a case review of patients with primary immunodeficiency and immunodeficiency-associated vaccine-derived polioviruses (iVDPV) detected between July 2018 and December 2019 from five countries (reported countries: Argentina, Egypt, Iran, Philippines, and Tunisia), 16 patients including two girls aged 9 month and 7 months and three boys aged 8 months-5 years [not all ages and sexes stated] were described, who developed iVDPV infection following administration of poliovirus-vaccine-inactivated or poliovirus-vaccine-live-oral. Additionally, acute flaccid paralysis secondary to vaccine-derived poliovirus infection were observed in 7 patients [dosages and durations of treatments to reaction onsets not stated; not all routes and outcomes stated]. This case review was reported in MMWR and Morbidity and Mortality Weekly Report of the Centers for Disease Control and Prevention. Of the 16 patients, 15 patients had a history of primary immunodeficiency including agammaglobulinemia, combined immunodeficiency disorder, major histocompatibility complex II deficiency, severe combined immunodeficiency disorder, B-cell deficiency or hypokalemia and infectious diarrhea and one patient had unknown primary immunodeficiency. The patients received the following poliovirus vaccination: 2 doses of poliovirus-vaccine-inactivated [inactivated poliovirus vaccine] and one dose poliovirus-vaccine-live-oral [bivalent oral poliovirus vaccine; 1 patient], three dose of poliovirus-vaccine-live-oral [trivalent oral poliovirus vaccine; 1 patient], poliovirus-vaccine-inactivated (1 patient) and poliovirus-vaccine-live-oral (13 patients). In between 15 July 2018 and 29 August 2019, 1–3 serotype iVDPV were detected (iVDPV infection) in all the patients. Seven of these 16 patients had acute flaccid paralysis secondary to iVDPV infection. At the time of report, 4 patients had died [immediate cause of death not stated], 6 patients had stopped excreting iVDPV, 6 patients were alive and were excreting iVDPV. Macklin G, et al.
