A universal fear of those who perform cataract surgery is posterior capsule rent. Although it doesn’t top most of our lists as the highest anxiety producer, it’s solidly on the list. We fear it, not so much because it is a terrible thing in and of itself, but because it travels in the company of so many other unpleasant things. We dread the unpleasant conversation we will have with the patient and their family after the surgery. We dread explaining “What went wrong?” and why this never happens “to any of my friends or relatives.” It is all very unpleasant, even though the experience of posterior capsule rent is a common one. All of us have been there, at least those who have done more than a handful of cataract operations. We feel personally responsible for this particular complication, and most of the time we are. Of course, we are not to blame for posterior capsule dehiscence in the occasional posterior polar cataract, or for iatrogenic tears produced by our vitreoretinal colleagues, or for capsular holes in eyes with a history of penetrating ocular injury.
