Potassium-hydroxide Severe irritation: case report A 70-year-old man developed severe irritation during treatment with potassium hydroxide for Molluscum contagiosum. The man, who had a history of allogeneic peripheral blood stem cell transplant for myelodysplastic syndrome, was referred for the analysis of numerous, new, pruritic papules on his trunk. He was diagnosed with Molluscum contagiosum, and started receiving treatment with 10% potassium hydroxide solution [route not stated]. However, within 2 weeks, this therapy was withdrawn because of severe irritation. Subsequently, the man’s therapy was changed to tretinoin, which was used for several months with no improvement. During this time, he continued developing numerous new lesions, which spread to other areas on the trunk and lower extremities. Due to this disease progression, he was initiated on Candida immunotherapy. By the time of follow up visit, approximately one month following the last Candida therapy, his lesions had completely resolved [outcome of the ADR not stated]. Author comment: "[M]ore severe and recalcitrant molluscum infection has been reported in adult transplant patients. . .Commonly utilized treatment modalities include. . .topical potassium hydroxide solution. . .which have demonstrated variable efficacy. . .A major limitation of these therapies is the potential for associated side effects, such as severe irritation and secondary scarring".
