Pramipexole Cluster headache: case report A 60-year-old man experienced a recurrence of cluster headache (CH) after starting pramipexole treatment for restless legs syndrome. The man presented with restless legs syndrome (RLS), which had been ongoing for several years, resulting in significant daytime sleepiness over the last year. Seven years ago, he had experienced episodic CH over a 9-month period; the CH had recurred about three times/day and had been right-sided, with ipsilateral lacrimation, nasal congestion, conjunctival injection, Horner’s syndrome and rhinorrhoea. He started treatment with pramipexole 0.18mg [route and frequency not stated] for the RLS. Three days later, he experienced a recurrence of CH with similar symptoms to the previous episodes. Pramipexole was discontinued after a few days. The intensity of the CH diminished over 3 days and had disappeared after a week. At the 18-month follow-up there had been no recurrence of CH. Author comment: "The sharp temporal relationship between the course of CH attacks and that of treatment initiation and discontinuation argues against a fortuitous association." Ossemann M.
