Preventing medication errors involving smart infusion pumps Medication errors involving smart infusion pumps, and strategies for preventing the errors, have been compiled based on results of systematic literature review and published in Drug Safety. MEDLINE, PubMed, PubMed Central, and Cumulative Index to Nursing and Allied Health Literature (CINAHL) databases were searched for articles on human-based medication errors associated with smart infusion pumps published during 2000–2015. The search results were later updated with searches of CINAHL and PubMed from 2000 to 2019. Identified error types and prevention strategies were catalogued. In total, 105 srticles were reviewed, and 18 error types and 10 prevention strategies were identified. The five main categories of errors were undocumented orders (undocumented verbal orders or unauthorised medication), drug library errors (wrongly selected, insufficient or by-passed), programming errors (wrong concentration, volume, dosage, infusion rate or bodyweight), administration errors (wrong patient, route, technique or drug, or missed dose) and ancillary equipment errors (wrong line through pump or wrong syringe size). Strategies to prevent smart infusion pump-related errors included interfacing smart pumps with electronic health records of patients, maximising the use of barcoding, improving compliance with drug libraries, standardising and updating drug libraries, optimising training and education on smart pump use, and standardising smart infusion pumps. "We intend this compilation and mapping of prevention strategies to serve as a toolkit for clinical use. Future research should investigate best practices associated with smart pumps and quantitatively assess the effectiveness of mitigation strategies developed to prevent medication errors and the application of the resources provided by this review," said the authors.
