Acute promyelocytic leukaemia differentiation syndrome: case report A 14-year-old girl developed acute promyelocytic leukaemia (APL) differentiation syndrome after receiving anaesthesia with propofol and sevoflurane [routes, dosages and time to reaction onset not stated]. The girl, who had high-risk APL, was undergoing unspecified surgery. She was clinically well without any respiratory symptoms prior to broviac line placement, and had an uneventful intraoperative course. In the post anaesthesia care unit, symptoms of respiratory distress occurred. She developed hypoxia, and a chest x-ray demonstrated bilateral pulmonary infiltrates. She was clinically diagnosed with differentiation syndrome, which was temporally related to exposure to the anaesthetic agents she had received, propofol and sevoflurane. After dexamethasone treatment and initiation of chemotherapy the girl’s respiratory symptoms resolved. Author comment: "We present a 14 year old female with high risk AP: (WBC 16 K, platelets 18 K) who developed [APL differentiation syndrome] immediately post anesthesia and prior to the initiation of therapy".
