Anaphylaxis: case report A 25-year old man with Achilles tendinitis developed anaphylaxis while receiving ranitidine [Zantac]. The man took oral ranitidine 75mg and aceclofenac, and developed itching, throat constriction, dyspnoea and erythema 15 minutes later [therapeutic indication not clearly stated]. He was admitted to an emergency room, and a drug reaction to aceclofenac was presumed. The man received an IV infusion of methylprednisolone and ranitidine 50mg, the latter for the prevention of peptic ulcers. He lost consciousness and developed a rash a few minutes later. His radial pulse was undetectable, and his breathing stopped; he received epinephrine [adrenaline] and was ventilated. Subsequently, his BP normalised and he regained consciousness. A skin prick test with ranitidine performed 3 weeks later showed an intense reaction, followed by headache, dyspnoea and an erythematous swelling of his ear lobes. He had no other drug allergies or any antecedent respiratory symptoms [outcomes not stated]. Author comment: "The immediate response to the skin prick test suggests that ranitidine was the drug responsible for the clinical reaction, possibly produced by an immediate-type hypersensitivity reaction."
