Acute renal failure and anaphylactic shock in an elderly patient: case report A 69-year-old woman developed acute renal failure and anaphylactic shock following treatment with rifampicin for suspected tuberculosis. The woman had initially received rifampicin, isoniazid and ethambutol for presumed tuberculosis; liver dysfunction, skin rash and gastroenteric trouble ensued, and treatment was withdrawn. Rifampicin [route and dosage not stated], isoniazid and ethambutol were reinitiated 3 weeks later following presentation to a tuberculosis clinic. She developed headache, chest discomfort, skin rash and gastroenteric trouble, a few hours after drug administration. Anaphylactic shock was noted following the onset of dyspnoea, mental confusion and hypotension. Epinephrine [adrenaline], steroids, and vasopressors were initiated, and the antituberculars were withdrawn. Tests revealed an elevated serum BUN/ creatinine level of 22.9/2.7 mg/dL; her urine output dropped by 5 mL/hour. Anaphylactic shock and renal failure continued for 2 days prior to stabilisation of her vital signs and normalisation of renal function. The results of a skin prick test lead to the conclusion that rifampicin was the origin of anaphylactic shock and acute renal failure; an induration, 13 x 12mm, was noted 30 minutes after rifampicin, 1.5 x 10-1 mg/mL, was injected. A new antitubercular regimen, excluding rifampicin, was initiated. Author comment: "Compared with continuous regimen, intermittent rifampin regimen has longer interval to accumulate more rifampin-induced antibodies, and more immunogenic side effects are the sequalae when reencountered with rifampin. In this case, we thought that patients with previous adverse reactions should be carefully monitored if reexposure to rifampin is essential." Ko Y, et al.
