Thrombocytopenia leading to intraparenchymal haemorrhage: case report An 81-year-old woman developed thrombocytopenia leading to intraparenchymal haemorrhage during treatment with rivaroxaban for atrial fibrillation (AF). The woman, who had hypertension and AF, was hospitalised with an acute right sided weakness, drowsiness, unresponsiveness to verbal commands and high BP. Twenty months prior to the presentation, she started receiving oral anticoagulant therapy with rivaroxaban 20 mg/day for AF. Neurological examination showed right-sided hemiparesis, right upper motor neuron- type cranial nerve VII palsy, impaired consciousness and sensory-motor aphasia. Brain CT scan showed intraparenchymal haemorrhage in the left hemisphere. Thrombocytopenia was also noted. The woman was treated with vitamin K, and rivaroxaban was stopped. Subsequently, the platelet count had normalised, and a repeat brain CT showed cessation of bleeding. Iliev D, et al.
