Interstitial lung disease as a paradoxical reaction: case report A 61-year-old man developed interstitial lung disease as a paradoxical reaction during treatment with secukinumab for psoriatic arthritis. The man had been receiving adalimumab for psoriatic arthritis with a good response for 9 years. However, his arthritis began to progress. Therefore, his treatment was switched to secukinumab [route and dosages not stated]. A good clinical response was noted. However, he developed a persistent cough, shortness of breath on exertion and blood-stained secretions. A CT thorax showed florid bibasal, lingula and middle lobe ground glass density air space opacification. Bronchoalveolar lavage was negative for atypical infection. Microscopy of bronchoalveolar lavage showed lymphocytosis. A diagnosis of interstitial lung disease as a paradoxical reaction to secukinumab was made [duration of treatment to reaction onset not stated]. Therefore, the man’s treatment with secukinumab was discontinued. Eventually, a significant improvement was noted. Saleh C, et al.
