Serotonin syndrome: case report A 38-year-old woman developed serotonin syndrome following concomitant administration of sertraline for post-natal depression and anxiety along with trigonella supplementation to aid lactation during breastfeeding [not all dosages and routes stated]. The lactating woman presented with post-natal depression and anxiety 6 months following birth of her first child. She reported to have increasing thoughts of harming herself and baby. Her medical history was significant for prescribed fluoxetine during her adolescence. She was admitted to an inpatient psychiatric ward and treated with sertraline 25mg on day 1 (while breastfeeding), followed by dose titration to 50mg the following day. She developed nausea, mydriasis, anxiety, diaphoresis, hyperreflexia, tachycardia and clonus following administration of second dose of sertraline, and required transfer to a medical unit. She was diagnosed with serotonin syndrome. Anamnesis revealed she had been receiving oral trigonella supplementation [fenugreek] daily to aid lactation. Serotonin syndrome secondary to potential interaction between sertraline and trigonella was suspected [duration of treatments to reaction onset not stated]. The woman received supportive care and was treated with diazepam to control symptoms. The serotonin syndrome self-resolved within 24 hours. Doolabh K, et al.
