SF-36 Definition This is essentially the same as the Medical Outcomes Study Short Form Health Survey – 36 (SF-36). The terms Medical Outcomes Study Short Form Health Survery - 36 can be used interchangeably with SF-36 which is used more commonly. To reiterate the previously define material, the SF-36 health survey is a standardized questionnaire with 36 questions used to assess patient health across eight dimensions including physical functioning, role-physical, bodily pain, general health, vitality, social functioning, role-emotional and mental health as well as psychometricallybased physical and mental health summary measures and a preference based health utility index. It is a generic measure, as opposed to one that targets a specific age, disease, or treatment group.
