Stroke: case report A 46-year-old man experienced a stroke after recreational use of sildenafil; the man had also been consuming alcohol. The man was hospitalised after losing consciousness for 12 hours, which had occurred 2.5 hours after ingesting sildenafil 50mg and 20–25 units of alcohol. He had a history of consuming 50–100 units of alcohol per week. He also developed right-sided weakness in his limbs, gait difficulties and global aphasia. Neurological examination was notable for hemiplegia and hyperactive deep tendon reflexes on the right side. Brain CT revealed an acute infarction in the left middle cerebral artery (MCA). Treatment with aspirin and nadroparin calcium was initiated with mannitol added due to mildly impaired consciousness on day 3 poststroke. During subsequent investigations, cranial magnetic resonance angiography (MRA) confirmed a left MCA infarction with recanalisation evident on repeat cranial MRA on day 11. Transoesophageal echocardiography was remarkable for a bicuspid aortic valve and patent foramen ovale, resulting in a right-to-left shunt. One month poststroke, the man was transferred to a rehabilitation centre. At this time, he had limited orientation and cooperation due to global aphasia, and required assistance to walk. Visual and oral communication was significantly improved 1 month later and at discharge he was able to walk independently using a tripod. Zinnuroglu M, et al.
